Network Case Management is looking to recruit experienced Clinical Case Managers to join our professional network on an agency basis. This role offers flexible working, competitive pay, and the opportunity to make a meaningful difference by supporting individuals with complex needs to achieve their personal goals. As a Clinical Case Manager, you will build strong and professional relationships with clients and key stakeholders, carrying out assessments, planning and implementing interventions, and reviewing progress to ensure positive outcomes. Work will be allocated in line with your availability and the current caseload.

As a Clinical Case Manager, your responsibilities will include:

  • Regularly meeting and communicating with clients to manage and progress their cases

  • Undertaking Initial Needs Assessments and reporting findings to relevant stakeholders

  • Working closely with a multi-disciplinary team to plan and implement strategies aligned with assessment outcomes

  • Managing time effectively to ensure appropriate support is provided to each client

  • Organising workload on a daily, monthly, and annual basis in line with agreed expectations

  • Reporting to your General Manager on timelines, progress, and outcomes for individual clients

To be considered for this role, you will need:

  • Excellent organisational skills with the ability to manage a varied caseload

  • Strong written and verbal communication skills, including report writing

  • Current HCPC registration as an Occupational Therapist or current NMC registration as a Registered Nurse, with currently practising Solicitors with relevant experience also considered

  • A minimum of five years’ practice as a Professional Registrant in your industry

  • Current registration or willingness to register with IRCM

  • A minimum of two years’ experience working as a Case Manager supporting clients with brain injury or complex needs

  • Membership of BABICM, VRA, or CSMUK

  • The ability to undertake an Enhanced DBS check for adults and children as appropriate

  • A strong working knowledge of relevant legislation, including the Mental Capacity Act 2005, the Health and Social Care Act 2008, and the Care Act 2014
