As a Clinical Case Manager, you will build strong, professional relationships with clients and key stakeholders, carrying out assessments, planning interventions, and reviewing progress to ensure positive outcomes. Work will be allocated based on your availability and the current caseload.

Key Responsibilities
As a Clinical Case Manager, your responsibilities will include:
Regularly meeting and corresponding with clients to manage and progress their cases
Undertaking Initial Needs Assessments and reporting findings to relevant stakeholders
Liaising with a multi-disciplinary team to plan and implement strategies aligned with assessment outcomes
Managing time effectively to ensure appropriate support for each client
Organising workload on a daily, monthly, and annual basis in line with agreed expectations
Reporting to your General Manager on timelines, progress, and client outcomes
Essential Requirements
To be considered for this role, you must have:
Excellent organisational skills with the ability to manage a varied caseload
Strong written and verbal communication skills, including report writing
Current HCPC registration as an Occupational Therapist, or current NMC registration as a Registered Nurse
Currently practising Solicitors with relevant experience will also be considered
A minimum of 5 years’ practice as a Professional Registrant in your industry
Current registration or willingness to register with IRCM
A minimum of 2 years’ experience as a Case Manager, supporting clients with Brain Injury or Complex Needs
Membership of BABICM, VRA, or CSMUK
Ability to undertake an Enhanced DBS Check (Adults and Children, as appropriate)
A strong working knowledge of relevant legislation, including:
Mental Capacity Act 2005
Health and Social Care Act 2008
Care Act 2014
